Registering Yourself

Your decision to donate your body to help others after your death is a compassionate gift! We are honored to assist you in this process. The forms needed to register your intent to donate are linked below. If you prefer to have information mailed to you, please submit a request for information and we will send a brochure and donation forms to you.

1. Download and complete the Donor Consent Form
2. Download and complete the Cremation Authorization Form
3. Download and complete the Death Certificate Vitals Worksheet
4. Mail your completed forms to:
   MedCure

   Portland, OR 97230
5. Keep a copy of your forms for your records.
6. Inform your family members about your decision to donate so they are aware of your plan and can help carry out your wishes upon your death.
7. Within 3 to 4 weeks of receiving your forms we will mail to you a Donor Card to carry with you.
