Registering Yourself

To register your intent to be a whole body donor in support of medical education and research for the future:

  1. Download and complete the Donor Consent Form.
  2. Download and complete the Cremation Authorization Form.
  3. Mail your completed forms to:

  4. Keep a copy of your forms for your records.
  5. Inform your family members about your decision to donate so they are aware of your plan and can help carry out your wishes upon your death.
  6. Within 3 to 4 weeks of receiving your forms we will mail to you a Donor Card to carry with you.

The Cremation Authorization form requires your signature to be made in front of a notary.  Notaries are usually available at your bank, or you can visit www.123notary.com for help in locating a notary service near you.
