Register Someone Close To Passing

body donation registration for person close to dyingA donor’s  legal next of kin  or  Power of Attorney for Healthcare can legally assist in arranging for body donation. A copy of the Power of Attorney for Healthcare document must be sent into us with the body donation forms.

  1. Download – Download and complete the Donor Consent , Cremation Authorization  and the Death Certificate Information forms. *Note: Cremation Authorization form can only be completed in advance of death by the donor or their Power of Attorney for Healthcare. Legal Next of Kin can complete after donor’s passing.
  2. Fax form – FAX your completed forms marked “URGENT” to: 503-257-9101
  3. Call MedCure – Call 1-866-560-2525 and speak with a Donation Coordinator to verify we have received your forms and to answer additional registration questions.

Registering Someone Who Has Passed

assist patient with completing registration for body donationIf your patient is receiving hospice care, please call our Pre-Registry Department at 1-866-560-2525 for assistance in completing the forms below and an interview to determine if there are any ‘red-flags’ to donation acceptance in advance.

Download and have patient or family complete the Donor Consent, Cremation Authorization and Death Certificate Information Form.
