Register Someone Close To Passing
A donor's legal next of kin or Power of Attorney (POA) can assist in arranging for body donation. POA document must contain language allowing to make final body disposition arrangements.
- Download – Download and complete the Donor Consent , Cremation Authorization and the Death Certificate Information forms. *Note: Cremation Authorization form can only be completed in advance of death by the donor or their POA. Legal Next of Kin can complete after donor's passing.
- Notary signature — The Cremation Authorization form requires a signature to be witnessed by a notary. Notaries are at local banks, or locate a notary service near you.
- Fax form – FAX your completed forms marked "URGENT" to: 503-257-9101
- Call MedCure – Call 1-866-560-2525 and speak with a Donor Coordinator to verify we have received your forms and to answer additional registration questions.
Registering Someone Who Has Passed
If a death has already occurred, please contact us immediately by phone 24 hours at 1-866-560-2525 and ask for a Donor Coordinator, who will ask questions to assist you further.
Registering My Patient
If your patient is receiving hospice care, please call our Pre-Registry Department at 1-866-560-2525 for assistance in completing the forms below and an interview to determine if there are any 'red-flags' to donation acceptance in advance.
